Please note that after you have completed this form, an e-mail will be sent to Jack Wyatt for confirmation. After your requested date has been confirmed available, you will receive 2 invoices via the e-mail address you provided. The first is your 50% deposit which must be paid IMMEDIATELY and the second is for the remaining 50% which must be paid at LEAST 30 days before your reservation.

Important things to note:

• 6 Person maximum/cottage
• No pets
• No smoking inside
• Must bring your own linens (1 queen 3 doubles per cottage)
• Kitchen utensils and cookware are provided
• 23" TV with CABLE /VCR/DVD is provided
